Job Summary

The Community Outreach Worker provides front-line social services including assessment, provision of information, referral, advocacy, supportive counselling and direct intervention to young adults-at-risk challenged by homelessness, marginality, drug dependence, mental illness.

Duties

• Ability to connect with and engage young adults
• Assess and identify client needs and develop individual service plans and follow-up
• Address social, mental health and dependency issues with young adults
• Coordinate and consult with other professionals, services and networks as needed
• Develop, implement and animate group activities and skill development workshops;
• Outreach to young adults through a variety of approaches
• Identify trends and community needs
• Fulfill administrative and data requirements
• Provide support services to families

Education, Knowledge and Skills

• Professional degree (Master’s level) in social work or other professional equivalent;
• Minimum of two years’ experience;
• Strong interpersonal skills
• Ability to balance professional and personal relationships
• Knowledge of social service network, government programs and services;
• Fluent bilingualism French and English; other languages an asset.
